text-indent: 2.1875em; box-sizing: border-box; visibility: visible;"><span leaf="" style="visibility: visible;">经济困难失能老年人集中照护服务，对入住养老机构的经济困难失能老年人给予救助，并对收住经济困难失能老年人的养老机构结合绩效考核结果予以适当补助。</span></p><p style="margin-bottom: 15px; text-indent: 2.1875em; box-sizing: border-box; visibility: visible;"><strong style="box-sizing: border-box;visibility: visible;"><span leaf="" style="visibility: visible;">（一）救助对象。</span></strong><span leaf="" style="visibility: visible;">救助对象为已纳入仙桃最低生活保障范围且自愿入住养老机构的失能老年人（经评估为中度失能、重度失能、完全失能的老年人）和高龄老年人（年龄在80周岁及以上的老年人）。实施长期护理保险政策地区参保人员已经通过基金支付基本护理服务费用的，不纳入救助范围。</span></p><p style="margin-bottom: 15px; text-indent: 2.1875em; box-sizing: border-box;"><strong style="box-sizing: border-box;"><span leaf="">（二）救助标准。</span></strong><span leaf="">我市养老机构收住经济困难失能老年人的收费标准，为我市本年度集中供养特困人员基本生活标准及全护理照料标准的总和（2024年城市特困人员基本生活费1061元/人/月，自理护理费455元/人/月，半自理护理费555元/人/月，完全不能自理护理费1967元/人/月，收费最高为3028元/人/月，农村特困人员基本生活费807元/人/月，自理护理费322元/人/月，半自理护理费422元/人/月，完全不能自理护理费1967元/人/月，收费最高为2774元/人/月，以后每年与仙桃市社会救助局公布最新标准挂钩），承接机构实行优惠服务，市社会福利院按照2200元/人/月标准向中度失能老年人及高龄老年人收取照护服务费用，按照3000元/人/月标准向重度失能、完全失能老年人收取照护服务费用。6所区域性集中供养机构按照1900元/人/月标准向中度失能老年人及高龄老年人收取照护服务费用。每名符合条件老年人享受的救助标准为入住养老机构实际收费标准扣除老年人已获得的最低生活保障金、残疾人“两项补贴”和经济困难的高龄、失能老年人补贴等行政给付后的差额。</span></p><p style="margin-bottom: 15px; text-indent: 2.1875em; box-sizing: border-box;"><strong style="box-sizing: border-box;"><span leaf="">（三）照护机构。</span></strong><span leaf="">仙桃市社会福利院和6所区域性集中供养机构为我市经济困难失能老年人集中照护承接机构，为有意愿入住养老机构的经济困难失能老年人提供集中照护服务。根据经济困难失能老年人集中照护需求，后续将符合条件的养老机构纳入集中照护服务承接机构。</span></p><p style="margin-bottom: 15px; text-indent: 2.1875em; box-sizing: border-box;"><strong style="box-sizing: border-box;"><span leaf="">（四）申请审批流程</span></strong></p><p style="margin-bottom: 15px; text-indent: 2.1875em; box-sizing: border-box;"><span leaf="">1.有入住养老机构意愿的最低生活保障家庭老年人，可向乡镇（街道）社会事务办申请进行老年人能力评估。</span></p><p style="margin-bottom: 15px; text-indent: 2.1875em; box-sizing: border-box;"><span leaf="">2.评估机构按照《老年人能力评估规范》（GB/T42195-2022）标准，组织对申请人开展能力评估，根据评估结果，评定老年人能力等级。</span></p><p style="margin-bottom: 15px; text-indent: 2.1875em; box-sizing: border-box;"><span leaf="">3.经评估确定为中度及以上等级的老人或年龄在80周岁及以上的老年人，向市民政局确定的集中照护机构提出集中照护申请，集中照护机构应在7个工作日内与申请人签订服务协议并办理入住。</span></p><p style="margin-bottom: 15px; text-indent: 2.1875em; box-sizing: border-box;"><span leaf="">4.在老年人入住养老机构满30日后，持养老服务协议和有效缴费凭证，向乡镇（街道）社会事务办申请救助，并填写《经济困难失能老年人集中照护等群体补助资金申请审批表》。</span></p><p style="margin-bottom: 15px; text-indent: 2.1875em; box-sizing: border-box;"><span leaf="">5.各乡镇（街道）社会事务办应对申请人已享受最低生活保障、残疾人“两项补贴”和经济困难的高龄、失能老年人补贴金额等情况进行初审，将相应资料于每月10日前报送至市民政局。</span></p><p style="margin-bottom: 15px; text-indent: 2.1875em; box-sizing: border-box;"><span leaf="">6.市民政局对申请对象实际入住养老机构收费标准、已享受补贴等情况进行审核。经审核符合条件的，作出予以救助的决定，同时确定救助金额（扣除老年人已获得的最低生活保障金、残疾人“两项补贴”和经济困难的高龄、失能老年人补贴等行政给付后的差额）。救助金从申请对象入住养老机构当月起算，并于次月支付到申请人账户。经审核不符合条件的，作出不予救助的决定，同时书面告知理由。</span></p><p style="margin-bottom: 15px; text-indent: 2.1875em; box-sizing: border-box;"><span leaf="">7.救助对象经济、身体状况发生变化导致可能不再符合救助条件或者引起救助金额调整的，本人、代理人、养老机构应当及时告知市民政局。市民政局将及时确认救助对象经济、身体变化情况，根据最新审核情况及时停发救助或者调整救助金额。处于最低生活保障渐退期内的对象，补助金继续发放至渐退期结束。退出低保范围的，自低保取消下月起停发补助金。取消补助后，如老人选择继续在养老机构内照护，产生的一切费用由老人自行承担。</span></p><p style="margin-bottom: 15px; text-indent: 2.1875em; box-sizing: border-box;"><strong style="box-sizing: border-box;"><span leaf="">（五）机构管理。</span></strong><span leaf="">收住经济困难失能老年人的养老机构应满足建筑、消防、食品安全、医疗卫生、特种设备等法律要求并具有收住完全失能老年人的服务条件。市民政局将定期对收住经济困难完全失能老年人的养老机构进行绩效考核，考核指标包括但不限于收住救助对象人数、救助对象满意度等，结合绩效考核结果对养老机构适当发放绩效补助，考核结果为优秀、良好的机构分别按实际发放基本养老服务救助金总额的30%、20%给予补助，考核结果不合格的养老机构取消其集中照护机构资格。</span></p></section></section><section style="justify-content: flex-start; box-sizing: border-box;"><span leaf="">（一）压实工作责任。市民政局、市财政局要履行牵头责任，加强救助资金使用和管理，细化业务流程、权责事项和工作规范，建立对相关养老机构的激励约束机制。市卫健委要加强老年人能力评估工作的实施和监管，督促市第一人民医院、各卫生院组建专业评估团队。各乡镇（街道）社会事务办公室要履行初审责任，确保救助资金使用和申请补贴资金的真实性，每半年开展一次全覆盖走访摸底调查。市民政局每年要按照不低于10%的比例对救助对象审核认定工作以及养老机构服务质量等进行随机抽查。要加强社会救助和养老服务业务协同数据共享和政策衔接，有序推进服务类社会救助发展，其中养老服务职能部门负责救助对象审核认定，指导养老机构提供相关服务，配合市财政局开展资金分配、支付和监管等工作；市社会救助局要做好最低生活保障审核认定和动态管理工作，同时会同养老服务职能部门确定经济困难老年人集中照护服务对象范围、认定条件、服务标准。</span></p><p style="margin-bottom: 15px; text-indent: 2.1875em; box-sizing: border-box;"><span leaf="">（二）严格资金监管。中央财政补助资金纳入困难群众救助补助资金统一监管。市财政局、市民政局对救助资金实施目标、支持对象、资金使用、信息公开等开展全流程监管和绩效评价，定期开展检查，确保资金管理规范和使用高效。市财政局、市民政局要高度重视资金安全，严格遵守财经纪律，确保原始凭证的真实性、审批程序的规范性、支付的合规性。对获得救助的困难失能老年人，民政部门应当在一定范围内以适当方式进行公示，并按规定建立中央财政支持经济困难失能老年人集中照护服务工作问题举报奖励机制，加强社会监督。对发生“套补、骗补”、虚报错报考核指标数据、违反财政资金使用相关规定、滥用职权、玩忽职守、徇私舞弊、优亲厚友等违法违规情况的，依照《中央财政困难群众救助补助资金管理办法》等规定依法依规严肃追究责任。对相关当事人骗取救助金的要依法追回。对养老机构与老年人恶意串通骗取救助资金的，纳入养老机构黑名单，实行信用惩戒，依法追究责任。</span></p><p style="margin-bottom: 15px; text-indent: 2.1875em; box-sizing: border-box;"><span leaf="">（三）提升照护质效。接收经济困难失能老年人入住的养老机构要在市民政局备案且符合《养老机构服务安全基本规范》强制性标准要求，健全完善管理制度，统一服务标准和规范，改善照护服务条件，不得对收住的经济困难失能老年人采取分灶吃饭、分区硬隔离等做法区别对待，不得影响现有集中供养特困人员服务水平和质量。养老机构需将救助对象入住和服务情况于入住后15日内录入全国养老服务信息系统，市民政局将利用信息化手段对养老机构服务质量和效果开展跟踪监测。</span></p><p style="margin-bottom: 15px; text-indent: 2.1875em; box-sizing: border-box;"><span leaf="">（四）强化政策宣传。市财政局、市民政局和各地社会事务办公室要主动做好政策宣传，发动村（社区）主动排查辖区低保家庭老年人，准确解读政策，将政策送至群众身边，鼓励社会力量举办的养老机构主动承担社会责任，引导慈善组织、志愿者、行业组织等参与经济困难失能老年人集中照护服务工作，对在工作推进中遇到的突出困难和问题要及时上报。</span></p><p style="text-indent: 2.1875em;
